The MO HealthNet Primary Care Health Home (PCHH) Initiative is working to provide care coordination and management for a medically complex population.
The goal is to improve the quality of life for both those providing care and those receiving that care. This cost-effective model supports the reduction of inpatient hospitalization, readmissions, and inappropriate utilization of emergency rooms for care, team-based care, active patient engagement in care, and improved clinical outcomes.
MPCA provides training and technical assistance to the 42 outpatient primary care organizations participating in the Primary Care Health Home of which 26 are CHCs.
