In addition to focusing on the patient, practice transformation also provides attention to the needs of the care team around purpose, meaning, and wellbeing.
By addressing the key components of practice transformation including goal setting, leadership, technical assistance, workflow changes, and continuous quality improvement; organizations can successfully navigate the process and position themselves to deliver high-quality, patient-centered care in alignment with evolving healthcare delivery models.
Primary Care Health Home
The Primary Care Health Home (PCHH) Initiative is pioneering a new approach to healthcare delivery, focusing on care coordination and management for medically complex populations. By promoting holistic care, the initiative aims to enhance the quality of life for both patients and healthcare providers.
Oral Health
MPCA’s Oral Health Network of Missouri provides education and training programs, access to resources and best practices, technical assistance, and advocacy efforts, to enhance the capacity and effectiveness of our members in delivering high-quality dental care statewide.
Behavioral Health
MPCA supports initiatives aimed at tackling pressing public health challenges head-on, including tobacco cessation, substance use disorders – including medications for opioid use, and harm reduction.
Value-Based Care
MPCA partners with our Clinically Integrated Network to provide training and technical assistance focused on driving health outcomes through data driven platforms, increased access to health information, and innovative patient engagement and outreach strategies. With the use of value-based care delivery models and programs, we have better positioned our health centers in reducing overall patient health care costs while improving access to and the delivery of quality care.
