Behavioral Health and Primary Care Integration strives to incorporate behavioral health services into the primary care setting. A key part of behavioral health and primary care integration is the addition of behavioral health consultants into the primary care team to improve access, clinical care, and collaboration between systems of care.
Behavioral Health Consultants support care team members in identifying and behaviorally intervening with patients to improve their physical health and chronic conditions, brief interventions for individuals with behavioral health problems, assistance with medication adherence, treatment plan adherence, self-management support/goal setting, and facilitate group classes.
MPCA supports health centers to expand integrated care to include tobacco cessation efforts, treatment for substance use disorders, including medications for opioid use disorder (MOUD) and harm reduction efforts.
