Care Manager
Amber Bryant, RN, CCM joined the Missouri Primary Care Association in 2023. She serves as the RN Care Manager for our Rural Health Cardiovascular Care Coordination Program. In her role, Amber works with Heather Centers and patients to strive for best patient outcomes and assist in meeting their needs.
Amber received her nursing degree from Lincoln University in 2009 and obtained her Certified Case Manager (CCM) certificate in 2020. Prior to joining the MPCA, Amber worked in hospitals as a bedside nurse focusing primarily on the pregnant, postpartum, and newborn populations. She has also worked in other roles with the Missouri Department of Health & Senior Services, and as a Case Manager for Managed Medicaid providers as well as for a virtual healthcare platform, assisting patients across all aspects of health.
